the program is the time it takes to set up
payroll deduction.

How
do I sign up my employees?
Just
4 Simple Steps:
Step
1: Select
the plan and print the below Applications and distribute
to your employees to complete.
Step
2: Print the Group Agreement
and complete.
Step
3: Print off Provider Lists
and Schedule of Plans and give to your employees.
Now
you have a group! (at least 3 must
participate):

4: Mail in the
following:
Your dental plan
will begin the 1st of the following month! Please note on a cover
letter to back date the group if applications are sent in after the
first of the month.
